Ilocos Norte Provincial Statistical Office has begun the rollout of the updating services of Philippine Identification System (PhilSys) for demographic information in the province of Ilocos Norte on 06 May 2024.

The updating service allows the registered individuals to change or correct their demographic information entries in their National ID/PhilSys ID such as name, sex, date of birth, place of birth, address, blood type, and any clerical and/or typographical errors. 

Client who wants to avail the updating service may now visit the PhilSys Fixed Registration located at G/F IT Bar-Annex Building, Valdez Center, Brgy. 1 San Francisco, San Nicolas, Ilocos Norte.

The PhilID card or ePhilID holder who will avail the updating service is advised to personally appear at San Nicolas PhilSys Fixed Registration Center. The client shall also accomplish the PhilSys Form No. 2. and shall present the applicable supporting document/s to request for the change in and/or correction of entry/ies in the demographic information. 

For the change or correction of Name, Sex, Date of Birth, Place of Birth, the registered person must present any of the following:

  1. PSA/NSO-issued Certificate of Live Birth or Report Birth;

  2. Annotated PSA/NSO-issued Certificate of Live Birth or Report of Birth (in case of administrative or judicial correction of entry/ies);

  3. Annotated PSA/NSO-issued Certificate of Live Birth or Report of Birth due to RA 9255 and legitimation by the subsequent marriage of parents;

  4. PSA/NSO-issued Certificate of Marriage; 

For the change or correction of Marital Status, the registered person must present any of the following:

  1. PSA/NSO-issued Certificate of Marriage;

  2. Certificate of No Marriage (CENOMAR);

  3. Advisory on Marriages bearing the necessary and appropriate remarks (CEMAR);

  4. Annotated PSA/NSO-issued Marriage Certificate due to annulment of marriage, declaration of nullity of marriage, or judicially recognized foreign divorce, ground for divorce as listed in P.D. 1083 – Code of Muslim Personal Laws of the Philippines;

  5. PSA-issued Certificate of Death of the Spouse;

  6. Court order on the Declaration of Presumptive Death of husband/wife;

 

For the change or correction of permanent/present address, the registered person must present the following:

  1. Barangay Certificate or Barangay ID stating the correct address;

  2. Proof of billing (for at least three months) with the name of the registered person; or

  3. Any supporting document indicating the correct address

For the change or correction of blood type, the registered person shall present a copy of the blood typing result. For the change or correction of mobile address and email address, the registered person does not need to present any identification and/or any supporting documents. 

Any change or correction of entry/ies of the demographic information of the PhilSys ID/ National ID shall entail the issuance of replacement PhilID to the registered person. A new PhilSys Card Number (PCN) will also be issued for the updated demographic information of the registered person. 

Upon processing of the request for correction or updating, the PhilID card of the client will be surrendered to the registration personnel. The client may be immediately issued with an electronic PhilID (ePhilID) with the corrected demographic information. 

Currently, the updating service in the province is only available at the Fixed Registration Center located at the first floor of IT Bar building, Brgy. 1, San Francisco, San Nicolas.
